Grade Level Transition Meetings

A successful start to next school year starts this spring with intentional and focused transition meetings between grade levels. These ideas and tools can help you get organized.

Transition meetings, when focused on data over “stories about kids” provide for continuity of learning, student support, and teacher efficacy. 

Start by scheduling 45-60 minute blocks of time for each grade band meeting (e.g. K-1, 1-2, 2-3, etc.). Be sure the time also works in your calendar as you will want to be part of the process.

Create an agenda like the following (Agenda Template available HERE):

Overview and purpose setting: 

Purpose: 

  • Ensure a strong start to the 2026-27 school year.

Agreements: 

  • Keep focused on student needs and strengths.
  • Stick to the facts (v. opinion)
  • Anything else that will help us show up our best for each other and in service of our students as well as honor the time we have
10 min

Whole Class Transition

Current grade level team shares whole group observations (e.g. academic and emotional strengths; instructional strategies that worked; gaps or unfinished learning; students who may need to be placed in separate sections)

35-40 min

Individual Student Transition (2-3 min/student)

  • Current performance level — reading, math, behavior
  • Relevant identifiers — IEP, ELL, etc. (you may need to set aside additional time to discuss students with IEPs)
  • What works with this student? (instructional strategies, engagement hooks, motivators, etc.)
  • What doesn’t work? (triggers)
  • Priority for skill development; any supports/interventions that may be needed
5-10 min

Receiving Grade 

  • Ask any clarifying questions
  • Patterns they notice based upon conversation
  • Adjustments they anticipate needing to make

Wrap Up

  • Identification of students needing intervention at the start of school; and/or students who qualify for summer school, as applicable.
  • 30-day priorities for receiving grade level
